Continue Care Service Plan
Terms and Conditions

1. Minimum Commitment
– Patients agree to a 12-month minimum commitment from the date of enrollment.
– Payments are automatically charged monthly, if monthly option is selected.

2. Cancellation Policy
– Early termination fee applies if the plan is canceled before the 12-month term is complete.
– The cancellation fee is $350.00 or the remaining balance of the 12-month term, whichever is less.
– 30 day written notice required for cancellation.

– In event new hearing aids are purchased:
– 30 day notice may be waived, any remaining balance will be waived, and the older technology will no longer be covered by the service plan.
– For annual plans, remaining months of coverage will be transferred to the new technology.
– With either plan above, should the patient wish to continue covering their previous technology and keep plans active, they may chose to waive cancellation as their discretion.

3. Automatic Renewal for Monthly Plans
– The plan automatically renews each year, beginning a new 12-month term, and cards are charged monthly, unless canceled in writing at least 30 days before renewal.

4. Non-Transferable & Non-Refundable
– The plan is non-transferable and non-refundable, even if hearing aids are no longer in use. To cancel plans, see the Cancellation Policy.
– Coverage will be transferred to hearing aids that are replaced under the original manufacturer Loss and Damage warranty.

5. Credit Card Authorization
– By enrolling, the patient agrees to maintain a valid credit card on file with merchant processor and authorizes automatic monthly charges.
– It is the patient’s responsibility to update any billing info promptly.

6. Missed Payments or Failed Charges
– If a payment fails or is declined, the plan may be subject to a $45 fee per month. To cancel plans, see our Cancellation Policy.

7. Continued Care and Coverage plans include:
Hearing aid cleanings, adjustments or reprogramming, Bluetooth support, in-house repairs, software updates, hearing aid supply allotment, hearing aid batteries (excluding rechargeable batteries or chargers), teleaudiology and/or remote care adjustments, loaner devices, repair processing fees, commercial moisture removal, and priority scheduling. Any service or item not stated is not included.

8. Device Ownership
– This plan does not include the cost of hearing aids—only ongoing care and services for existing devices.

9. Clinic Right to Modify Terms
– Allison Audiology & Hearing Aid Center reserves the right to change the plan terms, pricing, or scope of services at any time.

10. No Insurance Filing
– Service plans are considered self-pay and no insurance claims will be submitted on the patient’s behalf for included services.
